Your privacy matters
Privacy Statement
Yonge Street Mission (YSM) is a social service provider that addresses poverty in Toronto and works with vulnerable community members by providing services and programs such as Food bank, employment support, childcare, healthcare, and dental services.
As part of delivering its programs, YSM collects and uses personal information while respecting your privacy. YSM is responsible for all the personal information under its custody and is committed to transparency and accountability, taking all appropriate and reasonable steps to protect the personal information of our clients, volunteers, employees, donors, and board members. YSM ensures all employees, volunteers and students are educated on its privacy practices and statements. YSM’s Privacy Committee is responsible for ensuring the organization operates in accordance with applicable legislation and adheres to privacy principles.
This privacy statement is guided by the principles of the Personal Information Protection and Electronic Documents Act (PIPEDA) and the Personal Health Information Protection Policy Act (PHIPA).
Personal Information
For purposes of this privacy statement, Personal Information is any factual or subjective information, recorded or not, that differentiates or helps identify an individual. E.g. name, address, birth date, ethnicity, etc.
WHAT PERSONAL INFORMATION YSM MAY COLLECT AND WHY?
YSM informs individuals of the purpose of collection at the time it is collected and, with your consent, only collects the information necessary to provide services and conduct business.
YSM may collect the following information through the course of our work:
Participants/Clients may be asked for name, contact information, demographic data, financial and personal health information and observational notes to access food bank, healthcare, dental health, and mental health services, employment support. (Participants/clients may choose not to disclose any personal information that is requested. However, a decision to withhold some personal information can result in our inability to provide services.)
Information collection allows us to:
- assess eligibility for participation in YSM programs and services
- gain valuable insights to better understand our participant’ behaviours and needs
- develop well-informed goal setting programs for clients and participants
- empower us to adjust and improve our programs to achieve better outcomes
- mitigate risk and maximize efficiency in resource planning
- analyze and interpret data to prove or refine hypotheses, and provide impactful arguments in the poverty reduction sphere
- derive statistics to demonstrate YSM’s contributions to the community, enabling us to report findings to board members, current and potential funders and partners (data for stats and reporting purposes is anonymized and cannot be used to identify any individual)
Employees and prospective employees may be asked for their name, contact information, work history, resume, social insurance number, references and background check, and banking
information for hiring and recruiting process, payroll processing and establishing an employment relationship between the two parties.
Volunteers may be asked for their name, contact information, references and background check, and work history for the volunteer recruiting process and establishing new volunteer roles that support the mission.
Board Members may be asked for contact information, work/volunteer history for organizational requirements, board membership, tax and public reporting requirements.
Donors may be asked for contact information, demographics, gift history, credit card or bank information to process donations, process tax receipts, recognize and remind the continued importance of support.
Others: Individuals may be asked to provide their name and contact information if they wish to sign up for newsletters.
Exceptions: business contact information and certain publicly available information, such as names, addresses and telephone numbers which are published in telephone directories and are widely available to the public are not considered personal information.
HOW DOES YSM USE PERSONAL INFORMATION?
YSM only uses the personal information for the purposes originally identified or disclosed, unless otherwise consented to by the individual. This includes running programs and services, maintaining communication, managing relationships with participants/clients, employees, donors, and volunteers, process donations, managing volunteer programs and executing administrative tasks.
Personal information is not used for purposes unrelated to YSM’s mission without explicit consent.
Consent
YSM seeks informed consent from individuals before collecting their personal information. YSM properly informs the individual by clearly stating the purposes, uses, risks, and possible disclosures of the personal information, as well as provides an opportunity to ask questions and/or withdraw consent. Individuals are informed of the consequences of not providing consent, such as being unable to participate in certain programs.
In order to provide informed consent, the individual must also have the physical and mental capacity to consent.
Physical and mental capacity to consent:
- If there is a language barrier, interpretation will be provided by either a family member or an interpreter provided by YSM.
- An individual may lack the ability to provide valid consent. If an individual is determined to be unable to consent then a proper substitute decision-maker (e.g. family member, power of attorney) will be contacted to assess whether consent can be provided. All rights of an individual apply to a person who has been authorized to act on their behalf.
- An Individual who is unable to provide consent in one instance is not necessarily unqualified to consent in all instances.
In some cases, consent may be assumed, particularly when individuals seek our services and agree to their information being shared with other YSM staff members/volunteers/students who are directly involved in supporting them.
Expressed/explicit consent (written or verbal) will be obtained for any secondary uses of information collection (e.g. providing personal health information, including mental health, to non-health related YSM departments, providing information to external organizations).
Disclosure or Sharing
YSM does not disclose information for any purpose other than that for which it was originally collected unless explicit consent is obtained from the individual or as otherwise required or permitted by law.
Access to personal information is restricted to staff, volunteers, and students who provide services, are part of the circle of care (directly involved with community members on a ‘need-to-know’ basis), are assigned to tabulate or collate data, perform administrative duties, or require the data for research or educational purposes.
There are some situations where YSM may be legally required to disclose information without consent which include but are not limited to: subpoenas, search warrants, court orders, child maltreatment, college investigations, coroners, prevention of imminent harm (suicidal or homicidal intent), worker’s compensation board, infectious diseases, emergency situations, and to government agencies as permitted by law.
Cookies
YSM website uses cookies to enhance and improve the browsing experience and functionality of the website.
While YSM’s website makes use of cookies, it does not collect personal information. Any personal information site visitors voluntarily choose to provide (e.g. to receive electronic newsletters, participate in periodic polls and surveys, and/or to complete any of our online application forms for volunteering or receiving services) is not shared with third parties unless stated otherwise. Some third-party cookies on YSM’s website may be used to identify visitors for personalized and relevant advertising, site analytics, and other operational purposes.

Retention
Your personal information will be retained only as long as necessary to fulfill the purposes for which it was collected, and in compliance with any laws surrounding information retention. If all identified purposes for the use of your personal information have been exhausted, we will destroy the information or render it unidentifiable.

However, certain situations (including but not limited to compliance with federal or provincial laws, court orders, public interest purposes, or to protect the legal interests of our organization) may require YSM to retain and/or restrict the release of your data with you or third parties.
Security and Safeguards to Protect Personal Information
YSM commits to the security of information by applying necessary and appropriate safeguards with respect to your information kept on record. YSM uses multiple layers of safeguards such as administrative safeguards (e.g., need-to-know protocols), physical safeguards (e.g., locked file cabinets), and technical safeguards (e.g., access controls) to protect your personal information.

Individual Access and Individual rights
Individuals may request access to, correction of, completion of, amendment to, or deletion of their personal information, or challenge its accuracy, by contacting YSM at privacy@ysm.ca.
