Eligibility:
To qualify for Care Management support you must be 16 to 29 years old and experiencing multiple and complex barriers.
What to expect:
When you arrive you will be conncected to a Care Manger who will spend some time understaning your unique needs. They will recommend the services and supports that will provide you with the help best suited to you.
